Who needs a Second Class certificate
You need at least a Second Class medical certificate when exercising the privileges of a commercial pilot certificate in powered aircraft, and for Part 121 second-in-command operations that are not covered by the First Class requirement.
The exam protocol is the same as a Third Class exam with somewhat stricter vision standards. No EKG is required for a Second Class certificate.

Before you come in
File FAA Form 8500-8 through FAA MedXPress and bring your confirmation number, photo ID, corrective lenses, and a current medication list.
Dr. Edmondson strongly recommends reading the FAA's Know Before You Go beforehand, particularly if you have a new or ongoing medical condition.
If you are on a medication you have not previously reported, note it on your booking request. It is far easier to sort out documentation before the appointment than during it.
